At Reconnect4Health, we believe that all people deserve access to high-quality care and should be treated with respect, dignity, compassion, empathy and encouragement. What makes us different from other remote care companies? We believe that our human-centered approach is key, and our results show that patients and individuals enrolled in our remote care program are more apt to engage in their care when they develop relationships with their nursing team. As a nurse-owned and managed company, we are currently licensed in 43 states and plan to expand to all 50 states in 2022. Our expansion enables us to partner with health systems, home health agencies, technology solution companies, health plans & community-based program throughout the US.
