About
We are Health Care Professionals improving the quality of life for residents by increasing access, communication, and coordination of healthcare. We've raised the bar for expectations in post-acute and long-term care.
ICP is the only team of Nurse Practitioners that offers full-time coverage in a skilled nursing facility, and also provides on-call services. We work as a team alongside nursing facility staff to help meet goals for quality measures. For example, ICP Nurse Practitioners have helped dramatically reduce hospital readmissions by as much as 30%. And have seen results in less than 30 days.
We also work with facilities to reduce antipsychotic drug use, narcotic reductions, and polypharmacy management.
